    Physical information 

     The KHS Medical office will begin accepting calls for athletic physical exams on July 1st.  This year the physical exams will be completed at the KHS Medical Office and will be for fall sports only.

    COVID-19 protocols will be in place according to the guidelines for schools from the NYS Department of Health at the time of the physicals.

    We are encouraging athletes to get their physicals done by their own physician if possible as we have extremely limited appointments available.   All athletes that carry medications, such as, inhalers, epi-pens, those that need required immunizations, etc., and those needing COVID clearance, are required to see their own physician.  The school physician is not permitted to clear athletes after a COVID-19 infection without written documentation from their primary doctor, nor are they able to write medication prescriptions for them.   

      

    After July 1st please call 943-3742 to schedule a sports physical.   

  • 2021-2022 Sports Registration 

    Open to students that will be in 7th-12th grade this coming 2021-22 school year. 

    All athletes must complete the online sports sign up (click the REGISTER button below) in addition to the following steps. 

    Medical Paperwork:
    • Physical Form needs to be completed by a doctor.
    • Interval Form must be completed by a parent/guardian. (*This is a dated form - may not be submitted more than 30 days before the start of the season. Do not submit before July 23rd.)
    • Health History form must be completed by a parent/guardian. 

    • COVID-19 Clearance form must be completed by the student’s primary care provider if the student athlete has been diagnosed with COVID-19 in the past year.  

     

    Required forms are available in the links above and below or can be picked up at the Kingston High School Security entrance between 8am – 3pm, Monday – Friday. 

    Completed paperwork can be emailed to tigernation@kingstoncityschools.org or dropped off at the Kingston High School Security entrance between 8am – 3pm, Monday – Friday.
