Students may have the opportunity to attend off-campus activities for educational enhancement. To help keep your child healthy while away from the school building some requirements must be met.
1. There must be a signed parental consent on file for the trip (obtained by the student’s teacher).2. The school must have current and accurate emergency contact information (obtained by the student’s teacher).
3. If your child has current orders for medication while at school, the school nurse will work with you and your student to determine a medication plan for your child.
• Self-Directed: If your child is able to be self-directed when taking medication, his/her teacher may carry the medication for the student on the field trip so that the student can self-administer when needed. The school nurse will assess each student to determine if they can be self-directed according to the NYS guidelines(able to identify the medication, the number of pills or dosage to take, when to take the medication and how to take the medication).
• Non-Self- Directed: If your child is not able to be self-directed(not able to identify the medication, the number of pills or dosage to take, when to take the medication and how to take the medication)
A. The parent or parent designee (not a KCSD employee) may attend the activity and administer the medication or
B. The student’s doctor can write an order to:
• Adjust the time of administration.
• Eliminate the dose of medication for that particular event
• Write an order to suspend the order for all off campus outings, and resume upon return to school.Please click on the links below to access the forms that may be needed:
